List of Documents to be Submitted during Counseling
Following are the list of documents to be submitted at the time of counseling:
- AACCC Allotment Letter
- Admit card AIAPGET Entrance Examination
- AIAPGET Score Card
- Plus Two mark list
- Plus two pass certificate ( In case of CBSE or ICSE)
- A document to prove the date of birth. (10th Mark list)
- BAMS Degree Certificates
- BAMS Professional Mark list
- Internship Completion Certificate
- Registration Certificate
- Attempt Certificate
- Transfer Certificate from the Institution last attended.
- Migration Certificate from the Board from where the candidate passed his/ her BAMS.
- A Character and conduct Certificate from the Institution from where the student passed his/ her BAMS examination
- Medical Certificate for physical fitness and mental soundness from a medical Officer not below the rank of a Civil Surgeon or a clinical Associate Professor of a Medical College.
- Community Certificate in case of OBC, SC/ ST candidates.
- Certificate of Disability (if applicable)
- EWS Certificate (if applicable)
- NRI documents (if applicable)
- Photos – 8 Nos. Same as affixed in Ayush application form
- Passport size photograph of Local guardian – 2 Nos. (If there is no local guardian photo of Parent/Husband )
- Aadhar Card- Orginal & 1 copy (Proof of Identity)
- Full Fees DD: In favour of Amrita School of Ayurveda, Payable at Kollam.
